Hey, so I've been trying to play the game (obviously lol) and I have no idea what the stroke counter does, is it supposed to play a tick every time I stroke? or is it the number of times I can stroke during a slide? I'm really lost.

The stroke counter represents how many times you may stroke during a slide. But no need to keep track of it yourself: everytime you hear a tick you stroke once.
Different strokes for different people, but in this case a stroke is most often a move up and back down the shaft of your penis (down and back up is also possible.)

...as a default. Certain cards will specify something specific. As far as I can remember, no one has made a 'Stroke up then down, instead of down then up' (yet)
The tick is a metronome, just helping you maintaining your stroke rate!
You can always mute it if it bothers you, the beauty of CTI is that you are in control of your own game!
